Referral Process

Everything starts with a referral. Most often, a doctor’s office or hospital will make the referral however, they can come from anyone such as a neighbor, family, friend, or even the individual themselves.

Even if you are unsure if they are eligible, you are welcome to contact us and we will take it from there.
